Incident reports

An incident report is a dated record of something that went wrong: an injury, a bite, a behavior problem, or a condition you found during a groom. File one as soon as you can, while the details are fresh.

Ask Groomr

“I need to file an incident report. Max nipped Sarah during his nail trim this morning.”

Groomr takes down what you’ve told it and asks for whatever is still missing, one thing at a time:

  • the client and the pet
  • the type of incident
  • when it happened
  • what happened
  • how serious it was
  • whether the owner has been told, and if so when and how

It also notes any actions taken, such as first aid or a vet visit, if you mention them. When it has everything, it files the report and gives you a report ID. You can then view the report or download the PDF from the Incident Report pages.

If you stop answering, Groomr checks whether you’re still there, and later closes the form so you aren’t stuck in it.

Or do it yourself

File a report

  1. Go to app.groomr.ai/incident-report and select Start new report. There isn’t a menu link to it yet.
  2. Client & pet: search for the client under Search client, then choose the pet.
  3. Incident details: choose the Incident type, set the Date & time, write a Description, and pick the Severity.
  4. Follow-up: add Actions taken (optional), then answer Owner notified?. If you choose Yes, fill in When? and Method.
  5. Signature: have the owner sign in the box (Sign here (required)).
  6. Review: check everything, then select Submit.

Use Next Step and Back to move between steps. When it’s filed you see Report filed, with Download PDF, View report and File another report.

Find a past report

  1. On the Incident Report page, select Search past reports.
  2. Narrow by Date from, Date to, Type and Severity, then select Search.
  3. Select a result to open it.

You can also find a pet’s reports on their profile, under Care, then Incident reports. Each one has View and PDF links.

Give the owner a copy

  1. Open the report.
  2. Select Download PDF, then print it or send it to the owner yourself.

Good to know

  • Incident types: Injury to dog, Injury to person, Behavioral, Pre-existing discovered, Equipment/facility and Other.
  • Severity: Low, Medium, High or Critical.
  • Owner notified by: In person, Phone, Text or Email.
  • A filed report can’t be edited from its page. It’s a record, so it stays as it was filed.
  • Only the owner can use the Incident Report pages today. Filing, searching and downloading on these pages is limited to the owner, and there isn’t yet a way to give anyone else access to them.
  • Anyone can file through Groomr. Filing from chat works for every default role, so staff can report an incident even without access to the pages.
